We’re doing an evaluation, and we need you to tell us on what we can do to make your experience better. Whether you read over the Forums once every now or then, or post every day, your feedback is vital for the growth of building a strong and supportive community.

Terms and Conditions: SANE Forums Evaluation 2016

1. The Promoter of this competition is SANE Australia ABN: 92 006 533 606.
2. By entering the competition, you agree to be bound by these Terms and Conditions. Any instructions relating to the competition on the promoter's website form part of these Terms and Conditions.
3. The promoter may in its discretion refuse to award any prize to any entrant who fails to comply with these Terms and Conditions.
4. Entry into the competition is free.

ELIGIBILITY

5. Entry to the competition is open to residents of Australia.
6. An entrant's residential postcode will determine whether the entrant resides in the Australia for the purpose of this competition.
7. An entrant must be an individual and not a company or organisation. Employees of the promoter and any person associated with the competition, including a spouse, defacto spouse, parent, child or sibling (whether full, half, step or by adoption), are not eligible to enter.

COMPETITION PERIOD

9. The competition opens on 1 December 2015, and closes at 23:59 PM on, 31 January at 23:59. Any entry received after the expiry of the competition period will be deemed invalid. No responsibility is accepted for late, lost, delayed or misdirected entries.

HOW TO ENTER

11. To enter the competition, you must:

12. You cannot complete the survey more than once, if you do enter more than once all entries will be deemed invalid.

13. By registering your details at www.surveymonkey.com entrants must comply with SANE Australia's terms and conditions as set out at www.sane.org and www.surveymonkey.com

YOUR ENTRY

14. All entries become the property of the promoter.
15. To be eligible for a prize you must complete the entire survey.
16. By entering this competition, each entrant authorises the Promoter to contact the entrant regarding their entry.
17. All entrants must ensure that their entries are received by the Promoter during the Entry Period. The Promoter takes no responsibility for late or misdirected entries or for any delays or failures in any delivery service or equipment.

PRIZES

18. All entries in the competition go into a random draw. The draw will be held at 12:00 PM EST on 8 February, at SANE Australia, 120 Clarendon Street, South Melbourne, VIC. The promoter will randomly select four winners from the draw.
19. Prize winners will receive: 1 of 4 $50 Coles/Myer gift voucher.
20. A winner’s right to receive a prize is not transferable or exchangeable and cannot be taken as a monetary payment.
21. The promoter shall not be responsible for any additional costs associated with use of the prize.
22. If the prize is date specific e.g. a use-by or redeem-by date, the prize winner must redeem the prize before the dates stipulated on the prize. If the prize winner does not redeem the prize before the valid date, the promoter takes no responsibility for reimbursing or reissuing the prize.
23. No component of the prize can be transferred or redeemed for cash.
24. In the event that any of the prizes become unavailable due to circumstances beyond the promoter's control, the promoter reserves the right to provide a similar product to the same or greater value as the original prize, subject to any applicable laws or written directions made under applicable legislation.

WINNER NOTIFICATION

25. The prize draw will occur within 30 days of the competition closing date. The prize winners will be notified within 2 days of the prize draw by mail, telephone or email.
26. In the event that the prize winner is unable to be contacted, the Promoter will retain the prize for five (5) days from the date of the draw. If the prize is not claimed within five (5) days of the date of the draw, the Promoter will redraw another winner for the unclaimed prize. The re-draw will take place at SANE Australia, 120 Clarendon Street, South Melbourne, VIC. Re-draw prize winners will be notified as set out in above.

GENERAL

27. You warrant that all details provided with your entry are true and accurate.
28. You agree that the promoter may use your personal details provided to the promoter for the purpose of conducting the competition.
29. If there is any event that prevents or hinders the promoter's conduct of the competition or the promoter's ability to deliver the prize/s to the prize winner/s, the promoter may, in its discretion, cancel the competition and recommence it at another time under the same conditions or select another winner.
30. The promoter is not responsible for any incorrect or inaccurate information, or for any failure of the equipment or programming associated with or utilised in this competition, or for any technical error that may occur in the course of the administration of this competition. The promoter assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorised access to or alteration of entries.
31. To the full extent permitted by the law, the promoter will not be liable for any death, injury, damages, expenses, or loss whatsoever (whether direct or consequential) to persons or property as a result of any person entering into the competition or accepting or using any prize, including without limitation non-receipt of any prize or damage to any prize in transit and the prize winner's failure to comply with the terms and conditions (if any) specified by any third party
